The Home Dialysis Nurse is the clinical backbone of the Cooper care team. This role combines advanced clinical assessment, patient education, and independent decision-making — all delivered in the patient's home. Our nurses lead in-home training for both PD and HHD, validate patient competency, monitor clinical outcomes, and serve as the primary liaison between the patient and their nephrologist. Travel to patient homes throughout the assigned region is a core and daily part of the role.

Requirements:

• Active, unrestricted Registered Nurse (RN) license in the State of Florida.

• Minimum of one (1) year of home dialysis experience (HHD and/or PD).

• Demonstrated knowledge of CMS ESRD Conditions for Coverage.

• Strong clinical assessment, documentation, and regulatory compliance skills.

• Valid Florida Driver’s License and ability to travel to patient homes.

• Current BLS certification.

The Patient Care Technician is a field-based, patient-facing role centered on compassionate in-home support. PCTs reinforce patient education, assist with approved technical tasks including lab specimen collection and equipment support, and serve as a trusted presence in the patient's home. This role requires high emotional intelligence, strong communication skills, and a genuine ability to connect with patients and families in their own environment. Extensive daily travel to patient homes is a core component of the position.

Required:

• Patient Care Technician (PCT) certification.

• High School Diploma or GED.

• High emotional intelligence (EQ) with demonstrated empathy, compassion, and interpersonal awareness.

• Strong verbal and written communication skills, including the ability to confirm understanding and adjust communication style.

• Ability to organize work, follow through on commitments, and demonstrate accountability.

• Comfort learning and using computer systems, including Electronic Medical Records (EMR), Microsoft Outlook, Word, and Excel.

• Valid Florida Driver’s License and reliable transportation.

• Ability to perform regular field work with extensive patient home visits.

The Social Worker provides psychosocial assessments, counseling, and resource navigation to support patients receiving home dialysis. Working remotely and in coordination with the interdisciplinary care team, this role addresses the behavioral health needs, social determinants of health, and caregiver dynamics that directly affect a patient's ability to succeed on home dialysis. The Social Worker also supports transplant readiness, financial assistance navigation, and the emotional wellbeing of patients and families throughout their care journey.

Requirements:

• Master’s degree in Social Work (MSW).

• Active, unrestricted LCSW license in the State of Florida.

• Two (2) or more years’ experience providing psychosocial services, preferably in ESRD, chronic illness, dialysis, or behavioral health.

• Ability to comply with Florida Level II AHCA background screening.

• Proficiency with EMR systems and remote workflow.

• Strong communication, clinical assessment, and counseling skills.

• Ability to work independently in a remote environment.

• Attendance at in-person meetings as needed.

The Dietitian provides individualized medical nutrition therapy and ongoing dietary counseling to home dialysis patients across the Cooper caseload. Working as part of the interdisciplinary care team, the dietitian conducts nutritional assessments, develops patient dietary plans, monitors lab values, and helps patients navigate the complex dietary requirements of kidney disease — including phosphorus, potassium, sodium, fluid, and protein balance. The role is primarily remote with coordination through Cooper's clinical team and the patient's nephrologist.

Requirements:

• Master's degree in Nutrition, Dietetics, or a related field preferred.

• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.

• Active, unrestricted Licensed Dietitian/Nutritionist (LD/N) license in the State of Florida.

• Two (2) or more years' experience in renal nutrition, ESRD, or a dialysis setting strongly preferred.

• Ability to comply with Florida Level II AHCA background screening.

• Proficiency with EMR systems and remote workflow.

• Strong communication and clinical nutrition assessment skills.
