SMS Terms & Conditions

Program Description

Cooper Home Health operates an SMS messaging program to support patients enrolled in our in-home peritoneal dialysis (PD) and home hemodialysis (HHD) programs, as well as their authorized caregivers. Messages may include appointment reminders, care coordination updates, treatment support reminders, on-call support prompts, and educational content.

How to Opt In

You may enroll in Cooper Home Health SMS communications by:

  • Completing the SMS consent section on your patient intake form

  • Providing written consent to a Cooper Home Health care team member

  • Submitting consent through our online patient portal

Message Frequency

Message frequency varies based on your care program and schedule. You may receive up to 5 messages per week. Frequency increases may occur during active training periods.

Message & Data Rates

Message and data rates may apply depending on your mobile carrier plan. Cooper Home Health is not responsible for charges incurred from your mobile carrier.

How to Opt Out

You may opt out of SMS communications at any time by:

  • Replying STOP to any message from Cooper Home Health

  • Contacting your care coordinator directly

  • Emailing info@cooperhomehealth.com and by requesting removal

After replying STOP, you will receive one final confirmation message and will not receive further SMS messages unless you re-enroll. Opting out of SMS does not affect your care services in any way.

Supported Carriers

Most major U.S. wireless carriers are supported, including but not limited to AT&T, Verizon, T-Mobile, Sprint, Boost Mobile, and U.S. Cellular. Carrier support for SMS programs may vary.

Privacy

Cooper Home Health will not share, sell, or transfer your mobile number or SMS opt-in data to any third party for marketing purposes. See our full Privacy Policy at www.cooperhomehealth.com for more information.

Changes to These Terms

Cooper Home Health reserves the right to update these SMS Terms & Conditions. Material changes will be communicated to enrolled participants. Continued participation after notice of changes constitutes acceptance.
