Comparing Dialysis Care Models

There are three distinct models of dialysis care and they are not equal.

In-center hemodialysis requires patients to travel to a clinic for every treatment. Standard home dialysis programs let patients treat at home, but most still require regular clinic visits for labs, follow-ups, and check-ins. However, with Cooper Home Health, everything happens in the patient's home. No exceptions.

The Cooper Way

Standard Home Dialysis

In-Center HD

Dialysis Done In-Home

Every session is done at the dialysis clinic.

Zero Dialysis Clinic Visits

Cooper brings everything to you.

Regularly for labs, follow-ups, and check-ins.

Every treatment up to 3x per week.

All In-home Training

In-home Lab Draws

Usually requires lab or clinic visits

In-Home Follow-ups & Check-ins

Cooper brings everything to you.

Periodically in Clinic

In clinic and integrated into treatment visits

Dedicated 1:1 Care Team

Each patient gets an assigned care team.

Varies, often shared or with rotating staff.

Shared clinic staff across many patients

Patient-designed Schedule

Indeterminate, but more flexible than in-center

Clinic sets the schedule, typically daytime

Travel-free

Some travel still required for clinic-based care

Required for up to 3 round trips per week

24/7 Clinical Support

Periodically in Clinic

During clinic hours (After-hours varies)

Fewer Dietary Restrictions

Larger fluid shifts between weekly sessions

Strong Preserved Kidney Function

Continuous, gentle home treatment

Better preservation than in-center HD

Less preservation over time

We're a Comprehensive Home-based Care Model

What Cooper eliminates is the burden of traveling to a dialysis center for treatment, training, labs, and follow-up visits. As a patient, you never have to leave your home for any part of your dialysis care. Patients will still see their nephrologist and specialist team for ongoing medical oversight and that relationship never changes.

Comparing Modalities

Cooper offers both home dialysis modalities: Peritoneal Dialysis (PD) and Home Hemodialysis (HHD). They work differently, require different access, and suit different lifestyles. Here's an honest look at both — so you and your nephrologist can decide which fits your situation best.

Peritoneal Dialysis (PD)
Can be done during the day or overnight while sleeping.

Gentle

Needle-Free

How it Works:
Uses the natural lining of your abdomen as a filter. You can use a machine called a cycler or do manual dialysis without a machine, using only gravity to complete the process.

Access Needed:
A PD catheter — a soft tube placed in the abdomen through a minor outpatient procedure. No needles. The site heals within 2–4 weeks before treatment begins.

Treatment Schedule:
APD (Automated Peritoneal Dialysis) or CAPD (Continuous Ambulatory Peritoneal Dialysis). With APD, you connect before bed, disconnect in the morning, your entire day stays free. With CAPD, 3–4 manual gravity exchanges occur during the day, no machine or electricity required.

Training with Cooper:
5–10 days of in-home training with your dedicated Cooper nurse — aseptic technique, exchange procedure, exit site care, troubleshooting, and emergency protocols.

Best Suited for:
Daytime workers, parents, active patients who want treatment overnight. Also the strongest modality for preserving residual kidney function and for patients on the transplant waitlist.

Home Hemodialysis (HHD)
Flexible, around your schedule around your calendar.

Flexible Schedule

How it Works:
A dialysis machine filters your blood at home — same process as in-center HD, but on your schedule. More frequent, shorter sessions mean gentler, more thorough cleansing and better outcomes between treatments.

Access Needed:
An AV fistula (preferred) — a surgical connection between artery and vein, usually in the arm. Needs months to mature, so planning ahead is essential.

Treatment Schedule:
5–6 sessions per week, 2–3.5 hours each. Scheduled when it works for you — mornings, evenings, or weekends.

Training with Cooper:
3–8 weeks of in-home training with your dedicated Cooper nurse and care partner — machine setup, self-cannulation, alarms, troubleshooting, and emergency protocols. All in your home.

Best Suited for:
Self-motivated patients who want active involvement in their care. Patients with schedules that allow regular morning or evening sessions. Associated with excellent long-term outcomes and strong quality-of-life scores.
